New Oral Checkup Requirement
Any dentist, registered dental hygienist (RDH) or dental therapist licensed in Michigan can conduct the Kindergarten Oral Health Assessment. The assessment may be done through your local health department or within a private dental office. For more information, visit Michigan.gov.
Either a dental examination or a dental assessment done within 6 months of starting school will meet the requirements. Results must be documented on the PDF form below.
