Grandville Middle School, 4900 Canal Ave SW, Grandville

This is for any incoming 7-12th grade Grandville Public School student participating in sports next school year (2024-25).

$25 cash or check ONLY (checks written to Dr. Diana Dillman) 

Save time! Print off this MHSAA form, fill it out and bring it with you. You can also get the form at the GHS or GMS office ahead of time, or forms will be available at the physical as well.

Location: Grandville Middle School at 4900 Canal at the athletic entrance. Do not use the main academic entrance on 52nd.

If preferred, physicals can be done by your family physician. Bring the blank MHSAA form to your appointment, and turn in the completed form to the GMS or GHS office, whichever school your child will be attending in 2024-25.

As a reminder, physicals must be dated April 15, 2024, or after to be eligible for the 2024-25 school sports season.
