Participation Forms

The Grandville High School and Middle School Athletic Departments partner with FinalForms, an online forms and data management service that allows you to complete and sign athletic participation forms for your students. FinalForms saves data from season to season and year to year, meaning you will never need to enter the same information twice.

 

You may review your data at any time to verify it is current. You will be required to electronically sign your forms once per year and after any update.

 

Please note that your student’s annual physical/medical history form will still need to be turned into the athletic office.

 

We are asking that ALL parents of athletes use FinalForms. Follow these instructions to create your account, create your student's account and sign your forms electronically. If you require any support during the process, scroll to the page bottom and click "Use Support." Payment should also be made at this time. Free and reduced lunch participant’s pay-to-play fee is waived. Please call GHS or GMS athletic office to verify this information. There is a non-refundable 4% fee for online payments; however, the pay to play fee will be refundable if an athlete is cut or drops from a team prior to competition.
